Chemical Peels: Matching Depth to Skin Type Safely
A chemical peel is only as good as the match between the agent and the patient in front of you. The same solution that rejuvenates one face can trigger lasting pigmentation in another. Getting it right starts with two questions: how deep, and on whose skin.
The three depths
Peels are classified by how deeply they injure the skin. Superficial peels act within the epidermis using agents such as glycolic, salicylic, mandelic or lactic acid and address texture, dullness and mild pigmentation. Medium-depth peels reach the papillary-to-upper-reticular dermis and treat photoageing and fine lines; deep peels penetrate further still for more advanced changes [1][2]. Depth determines results, downtime and risk in roughly equal measure — deeper is not automatically better.
Fitzpatrick type drives the plan
Skin phototype is the single most important safety variable. Patients with Fitzpatrick types IV–VI carry a substantially higher risk of post-inflammatory hyperpigmentation, and deeper peels are generally avoided in darker skin for this reason [3]. With careful patient selection, priming and conservative depth, superficial and selected medium peels can be performed safely across skin of colour, but the margin for error narrows as phototype rises [1][3].
Prime, select, and screen
Good outcomes are set up before the peel. Priming the skin (for example with a retinoid or tyrosinase inhibitor where appropriate), screening for active infection, recent isotretinoin or a tendency to keloid, and confirming realistic expectations are all part of safe practice [1]. A test spot in higher-risk skin is a reasonable precaution.
Technique and aftercare
Standardised application, careful neutralisation where required, sun avoidance and clear aftercare instructions reduce complications and improve consistency [2]. When problems such as prolonged erythema or pigmentation occur, early recognition and management protect the result — which is why an algorithmic, protocol-driven approach is recommended over improvisation [2].
The training takeaway
Chemical peeling rewards judgement more than nerve. Knowing the depth–indication–phototype relationship, priming correctly, and stepping conservatively in darker skin will prevent the majority of complications [1][3]. The practitioners who consistently deliver glow rather than pigmentation are the ones who learned to read the skin first.
